Template for marine incident reports

In the event of even a small damage or accident on the club’s boats, the skipper prepares a marine incident report and sends it to the skippers mailing list under the heading “Meriselitys + boat + date“. The point here is that you can reflect afterwards how the damage could have been avoided, and everyone can learn from what happened together, so that the same incident does not occur again. Make all incident reports like this, it’s a learning curve and benefits everyone (and the club won’t judge you for an honest accident). If it would instead turn out that something has happened and no explanation has been made, the skipper will have repercussions. In case something happens, of course the situation will be taken care of first, and when the matter has been solved and situation is under control, notifications will be made as necessary to the head of boat maintenance and the next person who has booked the boat. Then within a few days, when there is more time to reflect on the events, a proper incident report should be written and sent to the rest of the skippers. A marine incident report is always made if something has happened to the boat or crew, or if it has hit ground. It’s also good to write about near-miss situations if someone could potentially learn something from it. Make sure to include the following:

Type of incident
Vessel the incident occurred on
Head of the boat at the time of incident
Location
Co-ordinates of the location
Date and time when the incident occurred
Water level
Prevailing weather conditions at the of the accident
Events leading up to the incident
Description of the incident
Reflection on how could I have prevented the accident with my actions as the head of the vessel? 
Reflection on which of the club’s or association’s safety instructions could have prevented the incident? 
Repairing the damage caused on the boat
